Please complete this form as thoroughly and accurately as possible, this will help us determine if the dog is a suitable candidate for our program.  Any inaccurate or incomplete information may be grounds for denial.  We are only accepting applications from North Carolina at this time.

Please understand that we are an all volunteer organization and that it may take 7-10 days to get back to you regarding your application.  In addition, once we decide to take a dog in it takes time to find a foster home and arrange transport.  Typically it is not an extremely quick process so if you are needing to rehome your pet within a very short amount of time (ie 2 days or 1 week), we may not be your most viable option.
